New Patients For new patients, please download, print, and complete the New Patient Packet and bring it to your first appointment in order for us to expedite your visit.

Appointments Please contact us via telephone or email to request an appointment. We strongly encourage all our new patients to register on the online patient portal and fill out the New Patient Packet ahead of time before your appointment.
What to Expect Please bring the following with you to your appointment.
- Health Insurance Card/Information
- Your Photo ID
- A friend or family member, if needed, to take notes/help with questions
- A list of all medications you are taking
- A list of your questions/concerns
- Copies of records from doctors related to your visit with us
- Register and completed forms on Patient Portal
Cancellation of an Appointment
Please notify our office at least 24 hours in advance if you are unable to keep your scheduled office appointment. If you must cancel a scheduled endoscopic procedure, we require that you call at least three working days (48 hours) in advance. Appointments and procedure times are in high demand, and early cancellation will give another person the possibility to have access to timely care.
How to Cancel Your Appointment
To cancel an appointment, please call (702) 483-4483. You may leave a detailed message on the voice mail if you are unable to speak directly with a receptionist.
Late Cancellation or No Show
Patients failing to cancel their office appointment as indicated above (at least 24 hours in advance) will be billed a cancellation fee of $50 if an initial consult and $25 for a follow-up visit. Patients failing to cancel their scheduled procedure as indicated above (at least 72 business hours in advance) will be billed a cancellation fee of $150.
All fees must be paid in full prior to the scheduling of future appointments.
Insurance and Billing
Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance claims for your reimbursement. Please remember that co-pays and/or payment for any non-covered services are required at the time of service. For your convenience, we accept cash, check, and most major credit cards (Visa, Mastercard, Amex, and Discover).

Facilities and Equipment
We perform most of our procedures at ambulatory care centers. We perform
certain procedures in the hospital setting. Innovative and highly advanced
tests such as Smart Pill Motility Monitoring System, Capsule Endoscopy,
Food Allergy Test, Hemorrhoid Banding, Anorectal Manometry, and Breath
Tests are performed in our office.
