Office Policy - Mask and Patient Only Allowed in Clinic
To our Valued Patients,
It is the current policy of CDIN, that all patients must wear a mask to cover from their complete nose to under their chin at all times when in our clinic, unless, you are asked by a team member to remove your mask. If you do not have a mask when you enter our clinic one will be provided for you. Please note everyone must adhere to our screening policy for fever, possible exposure or symptoms related to COVID to insure the safety of all. At this time, we are sorry; but we can only allow the patient in our waiting room, as well as; in the exam room. If you have a special need to have someone accompany you, please obtain approval prior to your appointment.
Office Policy - Unprofessional Behavior
The use of profanity towards our staff, disrespect of our CDIN Team and/or other guests or verbal/nonverbal actions in our clinic is not allowed. If you are inappropriate to our team or any individuals in our clinic and you impede the operation of the practice you will be asked to leave. We understand that these are exceedingly difficult times for all, but we cannot allow these behaviors. We are here to provide service and care to each of you, but we do ask that proper
